• Age Change up to or more than 5 years (Only once on foreign documents):

• 2nd Time Age Change (Only once and for 366 days):
  • Valid Pakistani or Foreign Passport or Resident Permit or Matric Certificate.
• 1 Year (366 days) Age Change:
  • 1 Year change is allowed on the basis of Birth certificate (Inland/Foreign).
• Unnatural age difference with sibling (less than 7 months):
  • Change up to one year (366 days) is allowed without any document, if applicant has un-natural age difference with other siblings.

Note: No document is required for addition of day and month in Date of Birth (e.g. 0-0-1970 to 5-7-1970)

• Complete Name change due to Religion change/correction:
  • Certificate from Dar-Ul-Afta / Islamic Centre (religion change from any to Islam only)
• Minor Name Change (Minor change means removal/addition of part of name i.e. Haji Muhammad Abid to Muhammad Abid):
  • Valid Pakistani Passport OR Foreign documents (Passport/ Resident Permit/ Work Permit/ Permanent resident Card/ Sojourn/ Travel document) with desired name OR Matric Certificate OR Undertaking for Name change (notarized by foreign local attorney/attested by Pak Mission) as per format available on the website.
• Partial Name Change (Partial change means removing/addition of cast only):
  • No document is required for partial name change

Note: Applicant neither can revert back to his/her previous name nor can modify name for 2nd time except female applicant (modified name with husband/father)

• Complete name change of parents:
  • MNIC (Old Manual National Identity Card) OR Old B Form (Manual) OR Old Passport (Pakistani) OR Matriculation Certificate (verified from concerned board) with desired parent name.

• Permanent Address Modification:
  Permanent address can be modified on provision of any of the following document:
  
  • Parents/siblings/children)/spouse’s ID cards with same address
  • Utility bill (Computerized electricity, gas or water) with same name and address.
  • If utility bill/blood relative/spouse registration document is not available, permanent address can be changed on the production of any property document on applicant / parents name.
  • Affidavit along with CNIC copy & property document from house owner
  • Verified Local/Domicile/PRC Certificate
  • Allotment letter on applicant/parent/spouse name.

Note: Present/Temporary address can be entered in permanent address on the basis of self or family member temporary address. Present/Temporary address on CNIC can be used as permanent address on NICOP. A temporary address mentioned on blood relative/spouse’s CNIC can also be used as permanent address on NICOP. No document required for change in present address if it’s a foreign address.

Once declaration / undertaking of being Muslim has been made on CNIC Form after 13 Dec 2018, then correction in Religion from Islam to other Religion is not allowed.

Note: Correction means If religion of the applicant is punched mistakenly as “Islam” previously which can be verified through family members, it can be corrected through declaration/undertaking.
